Non-Standardised Paperwork - Supervision Records and Case Reports
In addition to the above standardised documents, each course will use additional tools to meet important accreditation requirements.
Courses may use one of 2 options as determined by the course director:
A Reflective Therapy Record (RTR) to fulfil the above requirements which combines the weekly supervision notes, as well as sections that elicit detailed clinical information about the case and the course of therapy, including outcomes and reflections. No separate case report is required.
Complete a case report which gives the clinical details of the IPT delivered through the course of therapy and includes outcomes, reflections on the case. Additionally, weekly supervision sheets which includes goals and supervision questions, tracking and supporting progress using IPT model.
Helpful hint: Open the above docs in Desktop by clicking on the Editing button top right - this opens the Word version out of SharePoint. Go to FILE then SAVE A COPY for your own GDPR compliant version.
